Case Manager Duties and Responsibilities

Case Managers assist individuals living with mental health challenges through facilitating access to needed community resources and services through comprehensive assessment, service planning, referral, and monitoring. Case management services are recovery based services, intended to help participants gain (or regain) access to community resources (such as behavioral, medical, or social services) when unable to do so independently.

 

Minimum Requirements

  •  A Bachelor’s degree in a human service field (psychology, social work/services, etc.).
  • Pass a criminal history background check.
  • No more than 2 driving violations within past 3 years.
  • No DUI or reckless driving within 5 years.
  • Ability to work with diverse population who are experiencing a behavioral health crisis.
  • Ability to work independently as well as be team oriented (while much of our work is performed independently, we function and provide our services as a supportive team).
  • Flexibility and ability to manage ever-changing priorities.
  • Open to professional growth.

 *Preference will be given to experience in the mental health field, particularly agency type settings.

 

Duties and Responsibilities

  • Establish and maintain a professional relationship with clients. 
  • Meet with client and/or client's family to develop treatment plan (including goals). 
  • Establish a regular schedule to meet with client on a consistent and reliable (dependable) basis and work with client to develop skills that further them toward treatment goals.
  • Assist clients in the community, linking them to appropriate and effective community services.
  • Interface routinely with staff, Program Coordinator, Lead Supervisor, clinic staff, clients and other key personnel as identified essential to client care.
  • Complete twenty (20) hours of continuing education annually.
  • Maintain confidentiality of all client documents, files and communication in written, electronic and/or verbal form.
  • Assist in providing service delivery to clients experiencing a behavioral health crisis.
  • Work with clients experiencing behavioral health challenges.
  • Complete other relevant tasks as assigned by Supervisor 

 

Pay, Benefits, & Other Details

 

Job Type: Part-Time (to start) with option to build up to Full-Time. 

Salary: $19.00 - $22.00 per billable hour.

Education: Bachelor's (Required).

Experience: 1 year (Preferred but not required).
